Treatment of Acinetobacter Baumanii PDR in an Oldest Old Patient: A Case Report

Abstract

Background: Infections caused by pan-drug resistant (PDR) bacterias leave clinicians with very limited or no options for treatment. Oldest old patients are often ruled out of therapeutic possibilities due to their comorbidities, frailty and low glomerular filtrate. We report a case of an oldest old patient with pneumonia caused by Acinetobacter Baumanii PDR.

Case Report: A 97 years-old woman, with a medical history of heart failure and chronic kidney disease, stage V, was admitted to the Federico II hospital in Naples for dyspnea. Laboratory tests and X-rays showed clear signs of nosocomial pneumonia in our patient, who had recently been hospitalized twice. Cultural tests were positive for Acinetobacter Baumanii PDR. She was treated with a combination of Linezolid, Imipenem/Cilastatin/ Relebactam, dose based on glomerular filtration rate (eGFR), and corticosteroids. She made a slow but complete recovery from the pneumonia.
