By providing Medication Therapy Management (MTM), pharmacists optimize medication regimens, improve medication adherence, and educate patients on lifestyle modifications, leading to enhanced glycemic control and reduced HbA1c levels. Collaborative practices with healthcare teams help prevent complications and decrease hospitalizations, demonstrating the value of integrated care.
